Health and Safety Consent Information for Parents/Carers

Please read this information carefully. You will be required to provide your consent when booking a place.

Allergies

  • Parents/carers must provide MBF Sports with full details of any food allergies and any other known allergies affecting their child.

  • Parents/carers accept full responsibility for any allergic reactions that may occur as a result of foods or allergens that have not been disclosed to MBF Sports prior to attendance.

  • Parents/carers must inform MBF Sports of any severe or life-threatening allergies, including any asthma-related conditions.

  • All emergency medication, including EpiPens and asthma inhalers, must be supplied in its original pharmacy packaging. Each item must be clearly labelled with the child’s name and the prescribed dosage instructions.

Severe Life-Threatening Allergies

  • If your child requires an EpiPen or asthma inhaler, it is your responsibility to provide this medication in its original packaging with the prescription label attached.

  • All medication provided must be in date and unexpired. If these life-saving medications are not available on site, your child may not be permitted to attend the programme.

  • In the event of an emergency, you give consent for a trained member of the MBF Sports team to administer an EpiPen or other prescribed emergency medication where necessary. Emergency services will be contacted immediately following the administration of an EpiPen.

First Aid

Minor Injuries

You authorise a member of the MBF Sports team to administer first aid to your child in the event of a minor injury during the programme. You will be informed as soon as reasonably possible if first aid has been administered.

Administration of Medication

You consent to authorised MBF Sports staff administering prescribed medication to your child during the programme, provided it is administered in accordance with your written instructions and a completed Medication Administration Form. You will be informed before any medication is administered wherever reasonably practicable.

Emergency Medical Treatment

In the event of a serious medical emergency, you authorise any member of the MBF Sports team to call an ambulance and arrange any necessary emergency examination, treatment, or hospital admission. Every effort will be made to contact you immediately, and you will be expected to proceed to the hospital as soon as possible.

Sun Protection

Parents/carers are responsible for applying a broad-spectrum sunscreen with an SPF of 30 or higher to their child before they arrive at camp each day. Please also ensure your child brings a named water bottle and suitable sun protection, such as a hat, where appropriate.
